3. Pani/Pana dane osobowe (imię, nazwisko, email służbowy, tel. służbowy, instytucja) będą przetwarzane w celu uczestniczenia w spotkaniu informacyjnym na podstawie art. 6 ust. 1 lit. e (tj. interes publiczny) w związku z art. 11 ust. 2 i ust 4, art. 12 ust. 1 oraz art. 14 ust. 1 ustawy z dnia 5 czerwca 1998 r. o samorządzie województwa, a następnie w celach archiwizacyjnych, na podstawie art. 6 ust.1 lit c) RODO (tj. obowiązek prawny).
4. Pani/Pana dane będą udostępniane innym podmiotom, którym zlecono usługi związane z przetwarzaniem danych osobowych np. dostawcom usług informatycznych. Takie podmioty będą przetwarzać dane na podstawie umowy z Administratorem i tylko zgodnie z jego poleceniami.
5. Pani/Pana dane osobowe będą przechowywane do czasu zakończenia obowiązującego nas okresu archiwizacji tj. przez 10 lat, a potem materiały przekazywane są do właściwego archiwum państwowego.
6. Posiada Pani/Pan prawo dostępu do treści swoich danych osobowych oraz do żądania ich sprostowania, usunięcia, ograniczenia przetwarzania, wniesienia sprzeciwu.
7. Posiada Pani/Pan prawo do wniesienia skargi do Prezesa Urzędu Ochrony Danych Osobowych, gdy uzna Pani/Pan, iż przetwarzanie Pani/Pana danych osobowych narusza przepisy dotyczące ochrony danych osobowych.
8. Podanie danych jest warunkiem uczestnictwa w spotkaniu informacyjnym. Odmowa ich podania jest równoznaczna z brakiem możliwości realizacji ww. celów.

(3) Your personal data (name, surname, email, phone no., institution) will be processed in order to participate in the information meeting on the basis of Article 6(1)(e) (i.e. public interest) in connection with Article 11(2) and (4), Article 12(1) and Article 14(1) of the Act of 5 June 1998 on the self-government of the province act, and subsequently for archiving purposes, on the basis of Article 6(1)(c) GDPR (i.e. legal obligation).
(4) Your data will be shared with other entities contracted to provide personal data processing services, e.g. IT service providers. Such entities will process the data on the basis of a contract with the Controller and only in accordance with the Controller's instructions.
(5) Your personal data will be stored until the end of our applicable archiving period, i.e. for 10 years, after which the materials are transferred to the relevant state archive.
(6) You have the right to access the content of your personal data and to request rectification, erasure, restriction of processing, to object.
(7) You have the right to lodge a complaint with the President of the Office for the Protection of Personal Data if you consider that the processing of your personal data violates the provisions on the protection of personal data.
(8) Provision of data is a condition for participation in the information meeting. Refusal to provide them is tantamount to the impossibility of fulfilling the above-mentioned purposes.
